CARE MANAGEMENT
Care Management is a collaborative process of assessment, planning, facilitation and advocacy for service options and community supports. The goal of the Care Manager is to meet an individual’s health care needs through available services and community resources to promote quality cost effective outcomes. Care Management is intended to promote an individuals’ independence in their homes and to receive care in the least restrictive setting.
Who is Eligible?
Programs
All residents of St. Lucie County, Florida, age 60 and older, are eligible to use the services provided by the Council on Aging. A wide range of programs and activities is designed to appeal to seniors -- both the frail and the healthy -- who seek companionship, assistance, stimulation, and enrichment.
