CARE MANAGEMENT

Care Management is a collaborative process that includes assessment, planning, coordination, and advocacy to connect individuals with the services and community resources they need.

Our Care Managers work closely with seniors and their families to identify needs, develop personalized care plans, and ensure access to appropriate support services. The goal is to achieve the best possible health outcomes in a cost-effective way.

Above all, Care Management is designed to help individuals maintain their independence, remain safely in their homes, and receive care in the least restrictive setting possible.

Who is Eligible?

Programs

All residents of St. Lucie County, Florida, age 60 and older, are eligible to use the services provided by the Council on Aging. A wide range of programs and activities is designed to appeal to seniors -- both the frail and the healthy -- who seek companionship, assistance, stimulation, and enrichment.
